Every registration must include a completed Registration Form available from the Sign Up page

  • The form includes:
    • Designation of the primary contact and a secondary contact in matters relating to the website and information about your chief executive officer and billing contact.
    • Contact information must include an email address.
    • It is important that this information be current. If any contact information changes, please update us by email at support@digitaltowpath.org or by using the Contact Change Form available from the Forms section below.
